Abstract Aim The retromuscular repair described by Pauli et al. (2016) dissects the space, releases transversus muscle, and lateralizes stoma to place a mesh. aim was show results of robotic approach for this intervention. Material & Methods Video description case report. patient 71-year-old woman that underwent an abdominoperineal resection rectal cancer in 2017 presented with 5.8 cm parastomal hernia without midline defects (type III EHS classification). Results Three trocars were placed lateral right rectus sheath, posteriorly 12 mm assistance port inserted left hypochondrium. dissection began inferior preperitoneal until finding muscle. Then, sheath dissected superiorly laterally. A posterior release performed down–to–up direction, initially avoiding hernia, which later reduced. Posteriorly, peritoneum incised then sutured lateralize stoma. defect equally sutured. Finally, polyvinylidene difluoride (PVDF) mesh fixed transfascial sutures sides Conclusions totally extraperitoneal hernias is safe feasible, advantage intraperitoneal space.
